Information you provide will be held by SMASH. We’ll use it to process your referral and to manage our programmes. Just so you know, information you provide in here may be sensitive e.g. info about someone’s health and wellbeing. We only ever use this type of information when absolutely needed to understand someone’s needs and provide appropriate mentoring services. Children and young people can be referred by an adult who knows them well, for example a social worker, teacher or school nurse. Once a referral is received, it is assessed and you will be notified by our Atlas team.
